Social Services Designee

North Shore Health is seeking applicants for the Social Services Designee. The Social Services Designee is responsible for all social service activities within the hospital and care center; manages the operations of the social services department; provides social service assessments, coordination, and referral services to patients and residents; participates in providing staff education and support; participates in interdisciplinary teams and other committees as assigned.  The facility Social Services Designee must maintain a high level of professional knowledge and expertise in relevant program areas.

Minimum Qualifications (Preferred Qualifications)
Experience: N/A (Three (3) years in Human Services)
Education: High School Diploma or equivalent (Bachelor’s Degree in a Human Services field including, but not limited to, sociology, gerontology, psychology, etc.)
Licensure/Certification/Registration: Ability to complete a Social Services Designee Training Course (MN Social Work License)
