The Sawtooth Mountain Clinic is looking for a skilled and empathetic therapist to join our team of Behavioral Health Providers. We are seeking an LADC who is dually licensed as a LICSW, LMFT, or LPCC, as well.

Job Summary:
Provides diagnostic assessments and non-pharmacological treatment to patients of all ages with a wide range of mental health conditions in an integrated primary care setting. Works with the Behavioral Health Director, Chief Medical Officer, and a health care team including MDs, NPs, RNs, and MAs. This role includes professional collaboration as well as independent responsibility and accountability in decision-making.

Essential Duties and Responsibilities:
1. Assesses and diagnoses patients utilizing specialized knowledge, skills and assessment tools
2. Develops patient care/treatment plans based on defined area of clinical expertise, risk and overall patient care complexity
3. Provides outpatient therapy services as appropriate
4. Completes diagnostic assessments in accordance with standards of care
5. Identifies desired patient outcomes, evaluates and monitors patient outcomes and response to care
6. Plans, implements, and conducts patient/family education, as appropriate
7. Documents patient care in the clinical record according to clinical guidelines and professional standards, communicating significant changes in patient status and treatment goals
8. Assists providers/patients with acute mental health needs outside of scheduled appointments, when needed, consistent with current integrated behavioral health models
9. Consults with supervisory, care team and consultants, as needed and required
10. Contributes to the efficiency and effectiveness by participating as an active member of the patient care team
11. Participates in QI/QA activities in clinical care areas
12. Complies with organizational policies, procedures, standards of care and related laws and regulations, including HIPAA and OSHA regulations
13. Actively participates in staff meetings, BHI Team meetings, & quality improvement initiatives

11. Abides by corporate compliance program, HIPAA regulations, and other SMC policies and procedures
12. Utilizes Patient Portal to access patient information and communicate with patients, as relevant
13. Plans, organizes, and multitasks
14. Performs other duties as assigned

Job Qualifications:
1. Current Minnesota license as an LADC and one of the following licenses: LICSW, LMFT, LPCC, or Provisional License working towards one of the above licenses
2. Master’s degree or higher in psychology, counseling, social work, or marriage and family therapy
3. Minimum of two years’ direct patient care; experience in a primary care setting preferred
4. Team care model experience, patient-centered care philosophy
5. Proficient in use of computer software to document clinical care, and in use of Microsoft office applications
6. Knowledge of:
a. assessment, planning, implementation, and care coordination in mental health
b. standards for care/treatment plan development
c. patient rights principles
7. Skill in:
a. diagnosing patient conditions
b. interpreting findings from clinical evaluation and diagnostic tests
c. risk assessment and intervention

Physical Requirements:
• Mobility to work in typical office setting and use standard office equipment
• Stamina to sit or stand for extended periods of time, up to 8 hours per day
• Strength to lift and carry up to 20 pounds
• Corrected vision and hearing within normal range to observe and communicate with patients and professional staff, and to read printed materials and a computer screen
• Prolonged periods sitting, listening, and observing or working on a computer.
